Living with genital herpes can be frustrating, as outbreaks often require abstaining from sex and other activities until symptoms resolve. For this reason, you may wonder: how long does a herpes outbreak last?
First-time herpes outbreaks are typically the longest lasting, with symptoms resolving within about 2 to 3 weeks after onset.[1]
While genital herpes is a lifelong condition, the good news is that outbreaks tend to become shorter and less severe over time—especially with proper treatment and management.[1]
Herpes Outbreak Signs And Symptoms
The first herpes outbreak is often the most severe, though many people experience mild or no symptoms at all. Over time, some individuals begin to recognize early warning signs of an outbreak.
Common early symptoms (prodrome) include:
- Tingling or shooting pains in the lower body or genitals[2]
- Burning, itching, or discomfort in the affected area
- Pain or discomfort during urination
The most noticeable sign of an active outbreak is the presence of vesicles (sores or lesions), which may appear on or inside the:
- Urethra
- Penis
- Scrotum
- Vagina
- Vulva
- Cervix
- Anus or rectum
- Buttocks
- Mouth
In addition to sores, some people—especially during their first outbreak—may experience flu-like symptoms such as fever, chills, and headaches.[1]
Outbreaks are often more frequent during the first year after infection and tend to decrease over time.[1]
Can I Have Sex During A Herpes Outbreak?
It’s best to avoid all forms of sexual activity during an active herpes outbreak. This includes:
- Vaginal sex
- Anal sex
- Oral sex
- Kissing
Herpes is most contagious when sores are present, and engaging in sexual activity during this time significantly increases the risk of transmission.
Even outside of outbreaks, using condoms or barrier methods and communicating openly with partners can help reduce risk.[3]
Treating Genital Herpes: Outbreaks And Long-Term Management
Although herpes cannot be cured, it can be effectively managed with medication and the right care plan.
There are two main treatment approaches:
- Episodic treatment – Taken at the onset of an outbreak to shorten duration and reduce symptom severity. Common medications include:
- Acyclovir
- Famciclovir
- Valacyclovir
- Suppressive therapy – Daily antiviral medication used to reduce the frequency of outbreaks and lower the risk of transmission. In some cases, this can reduce outbreaks by up to 90%.[1]
Your treatment plan will depend on factors like outbreak frequency, severity, and medical history. Following your provider’s instructions closely is key to managing symptoms effectively.[4]
Treating An Active Genital Herpes Outbreak
While there’s no cure, there are several ways to manage symptoms and improve comfort during an outbreak:
- Keep the area clean – Gently cleanse the affected area and avoid fragranced products that may cause irritation
- Take soothing baths – Soaking in warm water for about 15–20 minutes can help relieve discomfort
- Wear loose clothing – Reduces friction and allows airflow to promote healing
- Use OTC pain relief – Medications like ibuprofen can help reduce pain and inflammation
- Identify triggers – Stress, illness, and hormonal changes may increase outbreak frequency[1]
- Test regularly – Routine STI testing helps you stay informed and protect your partners
